Houston Methodist Orthopedics & Sports Medicine will be conducting student physicals on Saturday, May 31, 2014 at Reliant Center in Houston from 9 a.m. to 12 p.m. for $15 (cash only). Free parking in Orange Lot with parking voucher (Enter through Gate 3).
Internal Medicine, Cardiology, Neurology and Orthopedic specialists will be on site to provide additional clearance as needed.
Pages 9 and 10 of the Lamar CISD Physical Packet and the Houston Methodist Consent form must be printed, filled out and signed by a parent or guardian prior to the physical.
Please bring these signed forms with you on Saturday, May 31.
