Verification of documents must be done prior to

your student attending his/her 1st day of school.

* Child's Certified Birth Certificate

*Child's Immunization Record

*Child's Social Security Card

*Proof of Address -Deed/lease Agreement, Utility bill (gas, water, electric) or voter's registration card.

*Parent/Guardian's State Photo I.D.
