Q: How do I get information on my elective products?

A: Contact First Financial Group of America (281) 847-8422.

Q: How do I change my beneficiary?

A:  To change your beneficiary on TRS you may contact payroll or retrieve the form directly from the TRS website. To update your beneficiary on your life insurance you must contact the Office of  Employee Benefits.

Q: How do I change my name and address?

A:  Your name may be changed by presenting proof of name change through an update social security card, marriage certificate, divorce decree or other legal documentation. The changes must be made in payroll for TRS as well as the Office of Employee Benefits.

Q: How do I know which tier my prescription is on?

You may go to myuhc.com and view the prescription tier levels.

Q: What is the plan year for Medical Reimbursement on my Flexible Spending Account?

A: The plan year begins October 1st and ends September 30th of the following year. However, a new federal law allows a participant to submit receipts for reimbursement through December 31st of the plan year.

Q: I have not received/I have lost my  insurance ID card how can I get a new card?

A: You may obtain a new card by logging onto www.myuhc.com, select <Manage My Account>, then select <Request ID Cards>. You may also print out a temporary card through the same menu options until you receive your plastic cards.

Q: Will I receive a card for my Spectera Vision Insurance?

A: You will not receive a card for your Spectera Vision Insurance. When you make your appointment with your vision provider advise your provider you are insured by Spectera. The provider has access to view your available benefits on-line without the need of a card.

Q: What is Bereavement Leave?

A: Berevement Leave is offered to LCISD employee's who have had a death in the immediate family. The employee is allowed five (5) days for a death in the immediate family. The five (5) days are paid only if the employee has accrued days to use, otherwise the employee will be docked. If you qualify for bereavement leave you must call your absence(s) in as "Local Sick Leave".

The term "Immediate Family" is defined as follows:

1.      Spouse.

2.      Son or daughter, including a biological, adopted, or foster child, a son- or daughter-in-law, a stepchild, a legal ward, or a child for whom the employee stands in loco parentis.

3.      Parent, stepparent, parent-in-law, or other individual who stands in loco parentis to the employee.

4.      Sibling, step-sibling, sibling-in-law.

5.      Grandparent and grandchild.

6.      Any person who may be residing in the employee's household at the time of death.

Q: Does LCISD accommodate employees who have been placed on light duty by their physician?

A: If an employee has been injured on-the-job the employee must report to the Office of Employee Benefits prior to reporting back to the workplace. The employee must provide a medical release to return to work. Every effort will be made to accommodate restricted duty for those employees who have been injured on the job. If you have not been injured on-the-job, you may only return to work with a release to full duty.

 

Q: Will my benefits continue for my dependent who has turned 19 years of age?

 

A: A dependent who turns 19 years of age continues to be covered if.....

      -the dependent is enrolled full time and has not graduated high school or

      -the dependent is enrolled full time in an institute of higher learning and has not turned 25 years of age.

If your dependent is between the age of 19 and 25 you must provide to proof of full time enrollment to the Office of Employee Benefits for each semester they are enrolled in order to continue coverage. This proof is due each September 15th and February 1st.

If your dependent has turned 25 years of age they are no longer eligible for coverage with LCISD.

In order to remove your dependent from your benefit deductions, you must complete an Election/Drop form at the Office of Employee Benefits. If you do not complete this form your deductions will remain the same until the form is completed; refunds will not be given if you do not timely drop a dependent.

Q: If I will not be employed by LCISD at the end of May when will my benefits end?

A:  The District will give you the option of when your benefits will end. You will be sent a form via interoffice mail with the option to receive a payoff on your mid last payroll. If you select the payoff option your benefits will end June 30th of that school year. If you do not chose the payoff option, you will have coverage through August 31st of the same year.

Q: Once I am hired when will my insurance benefits begin?

A: If you enroll in the health and dental plans your benefits will begin the 1st day of the month following your date of hire. If you enroll in any elective products these products will not take effect until the 1st day of the second month following your date of hire.NOTE:  Each new employee must attend a mandatory Employee Benefits Orientation prior to being allowed to enroll in any benefit offered.

Q: When is the plan year and when is the deductible year?

A: The plan year for enrollment is August of each year. However, your deductible year will run January 1st through December 31st. Therefore your deductible will begin again each January.
